Explanation:
Before it is too late, there is a need to point out that accommodation (or often in the plural, accommodations) has very little to do with Alojamiento, or lodging. I found this definition in Wikipedia:

An accommodation is a reasonable adjustment to teaching practices so that the student learns the same material, but in a format that is accessible to the student. Accommodations may be classified by whether they change the presentation, response, setting, or scheduling. For example, the school may accommodate a student with visual impairments by providing a large-print textbook; this is a presentation accommodation.

Of course my suggestion can probably be improved upon, but it should convey something along these lines.

When I was teaching in a university, every class would have students bringing me documentation of the ""accommodations they would need for their disabilities. Among these accommodations were more time for taking tests, permission to take tests alone in a little alcove, and posting of the lesson plan in writing on the board so that a student with auditory processing disorders could follow the flow the flow of the class.

Hi Neil. For the context I can definitely rule out "financial help" The form asks for information regarding the type of disability and how this one affects the student's performance/ Documentation of diagnosis , etc...Taking this into consideration makes me interpret "support for accommodation request" as in "justification for accommodation request" What do you think?

US institutes are so afraid of litigation for descrimination that their PC policies are way over the top.... I wouldn't be surprised if they have "support sevices for helping to complete forms" and that is what the text refers to.

Can you tell from your context whether it is either
a) "a request for FINANCIAL help/suppport with accommodation", or
b) "technical / administrative support for completing an 'accommodation request' "

I think it may the latter based on "To be used in consideration of...." i.e. this service (a support service) applies to request for accommodation.
If option (a) were correct (i.e. FINANCIAL support) then IMO the use of "To be used...." is a little strange - one doesn't really "use financial support"
