During the procedure the test material or sample is exposed to a complex of an enzyme linked to an antibody specific to the disease or substance being tested for; when the enzyme and the antigen-antibody complex bind, a color indicator or other easily recognizable sign occurs.

Na detekciu látok (antigénu) vo vzorke sa často využíva sendvičová ELISA, keďže test v takomto usporiadaní je veľmi citlivý a dajú sa ním identifikovať veľmi nízke hladiny antigénu. Na nosič (mikrotitračná platňa) sa naviaže protilátka (tzv. záchytná protilátka) k stanovovanému antigénu. Pridá sa analyzovaný antigén, pričom sa vytvorí komplex protilátka-antigén. Tento komplex sa ďalej deteguje ďalšou protilátkou (tzv. primárna protilátka), ktorá má na sebe naviazaný enzým (najčastejšie chrenová peroxidáza, alkalická fosfatáza, beta-galaktozidáza). Obidve protilátky, záchytná aj primárna musia byť špecifické k danému antigénu.
