IP is most likely a Latin abbreviation for "insufficientia [valvae] pulmonalis", i.e. "pulmonary insufficiency", more commonly called "pulmonary regurgitation" :-) It looks weird, as though the doctor didn't actually know Latin very well (which is actually pretty common).

Generally:
(1) Type of defect: R = regurgitation (less commonly: I = insufficiency, incompetence), S = stenosis
