The patient last visit to the clinic was on May 2013. He had no complaints from cardiovascular side. However epigastric pain he mentioned that he had mild epigastric pain which comes and goes at one stage while in Germany he had been diagnosed as ? a mildline abdominal hernia but it was small and he was offered no surgery.

I am afraid I did not understand the the meaning of the last two sentences. It was not written by a native speaker.
