English term or phrase: Post-nominal letters
Dear colleagues,
The context is an application form, where the applicant must fill out his/her details.
The cocerned field requires the applicant to add his/her (Title, full name, post-nomination letters).
Could you please give some suggestions as to the best way to translate post-nomination letters to Arabic? I do understand the meaning, but can't seem to be able to find an acceptable way to put it in Arabic.
Thank you all in advance.

Your text and the context play the decisive role in this case; for example if the application form is for academic higher studies then accordingly the terms may be more precise. This practically means you may translate ''post-nominal letters'' as ''الدرجة العلمية'' This may refer to BA or MA or Phd ...and ''title'' ''اللقب'' may refer to '''medical doctor' or engineer etc..I need more information and the context will give me clues .....
one thing is that it may be optional to fill out the field of ''post-nominal letters''. In this case it may be meant for a certain group of people in a coutry and culture different from yours, which means you leave it simply out; if it is of no use and does not apply.

Notes to answerer
Asker: Thank you for your help. But wouldn't it be awkward to translate the whole cell intoاللقب، الاسم بالكامل، الالقاب? Is there no other way to translate it so tgat it will be distinguished from "titles"?

Asker: Thanks a lot
