Explanation: het woord skanky klinkt sterker dan dirty zonder echt letterlijk naar een lichaamsdeel te verwijzen. Tyfushoer vind ik sterker uitgedrukt dan dirty whore.

There are two people, a man an a woman. In Burak's initial quote: 'Wat moest de tyfushoer antwoorden?' the woman ('allegedly a pornstar') is talking about herself, not about her male protagonist. Hence the confusion. This becomes clear when you read the 3 pieces provided by Burak in their correct order:
'(...) Zijn gezicht verstarde, zijn rechterwenkbrauw kroop omhoog van getergde verbazing. Ik zag dat hij agressie oppotte. Hij sloot zijn rechteroog en draaide met zijn hoofd alsof hij een stijve nek had. Hij maakte zijn brede bajesnek los, secondenlang. Toen opende hij zijn oog en keek me aan, zwijgend. *‘Leg jij eerst eens even uit,’ zei hij toen, ‘waarom jij in die rechtszaal was. Even if 'Wilbert' is an imaginary person it is still a man, however way you look at it. If this is all imaginary, then in her imaginary nonsense she is still describing an imaginary man as a 'tyfushoer'. 'Tyfushoer' still refers to a man. See?

You say the narrator is a girl. So the narrator in the passage you offered is the same girl, I suppose.
So again:

Certainly, 'he' refers to 'tyfushoer'
Of course in one passage a man can use the term and in another one a woman? That is, one time the female narrator uses the term herself and another time she has some personage use it

just out of curiosity, is the person being addressed a man or a woman? in that case you might not want to have the other person calling him a 'filthy whore'...
I suppose what we really need is some more context from Burak. When I see the words 'uitgeloogde oksels' used in a text, I don't really think we have to worry about offending sensitive 'Ladies' any more...;)
All those typically Dutch swear words involving horrible diseases like 'tyfus' and 'kanker' e.g. are particularly problematic. I grew up hearing things on the schoolplein in Hoofddorp that would really puzzle British ears if I were to try and translate them literally here in East Sussex today.
The point I'm trying to make is that this is not a word that would normally be used by a "lady" or a decent bloke for that matter and I would think very carefully before I use it in a translation. I do agree with writeaway, she hit the nail on the head.

it's easy to use really bad words in a foreign language because one never acquires the same sensitivity as a native speaker. I say things in French that I would never say in English. It just doesn't have the same effect on me-until I see the shocked look or hear the disapproving tone of French natives.....
I do know how it is frowned on, but my British fiancée has also been known to use it, but then perhaps jokingly. I have heard it used here in the UK in a variety of different contexts.
I think if Michael knew exactly how his second suggestion is frowned on in English speaking countries he would not have used it. I have been known to swear a lot due to my construction background, however this is a word that even I avoid, especially if there are any women within earshot.
