Wat is het voor een rapport? Als het een soort onafhankelijk deskundigenrapport is dan beoordeelt de arts dat litteken als storend. In dat geval zou ik ook denken aan ‘noticeable’.

Perhaps 'distressing' is an option after all, afhankelijk van hoe storend het is.

Iedereen heeft wel één of meerdere littekens aan de huid. Meestal geeft een litteken weinig problemen. Bij sommigen kan een litteken afhankelijk van het uiterlijk, de grootte en de lokalisatie, leiden tot ernstige cosmetische en psychosociale problemen.

Een litteken ontstaat wanneer de huid zich hersteld na een huidbeschadiging als gevolg van een ongeval, huidziekte of een operatie. Hoe meer de huid is beschadigd, hoe langer is het genezingsproces en hoe groter is de kans dat er een cosmetisch storend litteken ontstaat.

a phrase that seems to be used a lot is "unsightly" ("unsightly scars/scarring").
however, "unsightly" may be slightly too strong

if you could rewrite it, I'd go with something using the verb "to bother" ("the patient is bothered by a scar …")
