Explanation:
I must admit, I have no idea if this is a military / court martial form, criminal submission or civil defence pleading.

Reference

Reference information:

'Free will' heeft m.i. een veel bredere betekenis (filosofie, neurologie, zie bijvoorbeeld https://www.frontiersin.org/articles/10.3389/fnhum.2016.0026...

https://www.wolterskluwer.nl/INTERSHOP/web/WFS/WoltersKluwer... :

=====
=====
FYI: a 'wilsgebrek', i.e. de situatie waarin de wil tot een rechtshandeling gebrekkig is gevormd, is usually translated as 'vitiated consent' ("Consent may be vitiated by error, fraud, or duress").
=====
Indian law speaks of 'free consent' and has similar 'wilsgebreken' as Dutch law: => "What is Free Consent ? • Meaning of “Free Consent”[Sec.14] • A consent is said to be free when it is not caused by- 1.Coercion as defined in Sec15, or 2.Undue Influence in Sec.16 or 3.Fraud as defined in Sec.17,or 4.Misrepresentation as defined n Sec.18 or 5.Mistake, subject to the provisions of Sec.20,21 or Sec.22].
=====
Wilsvorming is obviously linked to wilsovereenstemming => consensus...
