In my perception of a non-native speaker, I'm not sure if 'limit from' sounds really right: IMO the choice would be between 'limit in' (being able to perform the action but with difficulty/limitations) and 'prevent from' (not being able to perform it at all).
Seems to me the BG source is itself a translation – or has been compiled under the heavy influence of similar foreign-language sources. Else, it should have been like:
Настоящото Ви здравословно състояние ограничава ли действията Ви (пречи ли Ви/затруднява ли Ви) при къпане или самостоятелно обличане?
or
Настоящото Ви здравословно състояние позволява ли Ви да се къпете или обличате сам/а?

Thus the question boils down to what the source is actually meant to imply. Maybe it would be best to ask the client?

There is a number of possible translations, but the asker has given a choice of two renditions. The difference between 'limit from' and 'limit in' is the difference between being able to do it at all and doing it badly.

I would go for IMPEDE. Zenithmaster also put it right.
