Strictly literally "hida" is fold or crease. Does kokoro have such thing? I think it does. What crease or fold creates? Shadow. Thus my translation. You interpreted it as "pain". In a way it is a pain, but not necessarily. It can be regret, it can be sorrow, it can be suppressed emotion of anger, frustration, despair.
Those are shadows in the heart.
