Re: increase the evening dose and decrease the daytime doses to reduce side effects during the day: I agree that there are drugs and situations where this concept works well (for example, with antihistamines for reducing daytime sleepiness). On the other hand, Artane is given in 3-4 divided doses and is therefore probably short-acting --> The patient would get no benefit from a high evening dose which improves his/her extrapyramidal movement disorder during the night. Neither is this strategy mentioned or recommended in the product information as a strategy for improving the performance of Artane.

That's why I feel the situation is too ambiguous to just go ahead and translate the usual meaning.

I agree with Mr. Martin and Ms. Neuhold, though I would suggest wording it a bit more in accordance with traditional 'medical speech' as "incrementally increase the dose". But, yes, it means to ramp up the dose, essentially. This is the incremental increasing of a dose for cautious monitoring of any side effects and to determine the threshold amount at which symptoms abate.

Example sentence(s):
  • This approach provides the patient with some personal control over analgesic dosing and can be used to estimate the increment in the fixed dose.
