Для Финляндии не нашла, но нашла для Канады, а канадская терминология часто ближе к европейской, чем к американской. Units на американском английском были бы названы offices. Я сначала подумала о бригадах, выезжающих для медицинской помощи на дому, но у наших units есть директор, менеджеры и т.п. Совсем не то. Думаю, что "службы" тут лучше, чем "отделы". Которые могут располагаться как в специальном доме, и тогда это больше похоже на дом престарелых, но может быть и вариант, что человек живет в своей квартире в обычном доме, просто к нему ходят соцработники вплоть до того, что приставлен личный помощник, который проводит с человеком много времени, если у того много разных потребностей.
