Se exceptúan de lo anterior, aquellas compras de bienes o servicios generales que se realicen en zonas alejadas y rurales, en donde no exista comercio formalmente establecido, o bien el número de proveedores sea marcadamente reducido, debiendo justificarse mediante comunicación en el soporte del Ck. 3. Monto de acuerdo al valor cancelado en cifras y letras
4. Nombre completo de quien presta el servicio, acompañado del número de cédula que le corresponda.

Por el contexto, no me parece que una traducción literal sea adecuada.

I don't think anything with "honor" is going to work in English. The whole idea of certification, as in certified translation, is that you're on your honor to tell the truth.

Explanation:
Looks like an Affidavit, but may be unsworn. A Deposition also needs to be sworn.

Otherwise, a 'Certificate of Hono(u)r' is an award or distinction granted.
