I am unsure which verb is leading this noun. Due to the length of the sentence I have tried breaking the sentence after "scellini", and beginning with... "However, an idea.....", but although I can easily translate what follows, I cannot integrate idea with any of the verbs. (I hope that's clear - if not do let me know).

"Gli elettori inglesi e perfino i fanatici del Premier conoscono le sue “boutades” e non gli conteranno gli scellini, ma una idea di come migliorerà o meno l’NHS da ora in poi se la faranno e possiamo star certi che questo diventerà una elle principali ,se non la principale,“issue” del futuro politico della Gran Bretagna."

Native Italian here! It is a tricky sentence.

The verb leading the "idea" would be "se la faranno". Another way of putting it would be "...ma se la faranno un'idea di come migliorerà o meno l'NHS".

The structure of the sentence "un'idea se la faranno" and the repetition of the pronoun suggests the writer is stressing the verb "farsi un'idea", meaning he's sure they will.

It means "it will give them some idea about how the NHS will (or won't) get better". The subject of "un'idea se la faranno" are "gli elettori inglesi e i fanatici del Premier". Let me know if this is more clear!
