Yes. The point is that, as David suggests, el servicio de urgencias al que alude "la guardia" need not be a hospital ER; it could also be a primary care / walk-in center where "urgencias" -"on the spot" medical attention with no previous appointment- are assessed by a medical team. I am not sure how to convey this broad idea in English. We call it "atención urgente" both at hospital and at primary care centers (servicio de urgencias de atención primaria / servicio de urgencias hospitalarias). Ingreso en guardia conveys thus that the report is an "emergency care report", but wonder if in English this expression would be used in a walk-in center...
Anyhow, the OP states this is a "long hospital report" so it must refer to an ER (emergency department) admission report. Maybe then ED report?

In the US, "guardia" in this case would refer to the MOD (medic on duty). When it is a surgical case, it's the SOD (surgeon on duty).

in Argentina if you feel there's something wrong and needing attention, you go to the "guardia" and they check you out without the need for a previous appointment

Ingreso a guardia parece expresión local (pej. Argentina) para referirse a admisión / acceso al servicio de ugencias. Entiendo así, por el título, que se trata de un informe de urgencias.
