Spanish term or phrase: modelo de acta
SPAIN. This appears in a description of job vacancy procedures for a research Institute. The "users" in this part of the instructions appear to be the assessment committee members who will evaluate the job applications. I don't think the usual translation "minutes" works here. So far I'm considering "report form"... (mainly because the forms I have to fill in for Social Security and Hacienda in Spain are referred to as "models").

Asker: Hi Taña. I'm not keen on "minutes" for this particular context. I've already translated "act" as "report" earlier in the document, as it refers to the report on the candidates made by the assessment committee members, rather than the minutes of the meeting, although they are partially synonymous.
