Am I right in thinking this simply means that the patient's deep tendon reflexes are normal?
The rest of the neurological findings are normal.

Still, you might include the information of medium intensity, as it may potentially be useful in the future: If this patient subsequently presents with symetrical weak DTR, previously reported average DTR would indicate pathological significance, whereas weak reflexes per se might well be normal.

The reflex is graded based on the amplitude of the response. The only reflex that is always abnormal is clonus. (The “+” after the number is to distinguish from muscle testing, it is not a meant as a “plus” or “minus” in the traditional sense.) Grades 1, 2, 3 are not by definition normal or pathological—they are depressed, average or elevated (Ferezy 1992).
In general, the grades are as follows:
1+ = present but depressed
2+ = normal / average
3+ = increased
4+ = clonus
