Hi,

I have seen this translated as Determinations or Findings in specialised contexts, and I could also imagine a translation like "Summarization/Rundown of Facts" but what do they really say in this slightly more specific context of a notarized Swiss set of MAA?

It is a headline in the notarization record and is also in other respects a somewhat significant term for the job.

Obviously, none of the data below is genuine.

The 2 reasons I narrowly avoided declaration(s) are 1. it is likelier to be the company promoters/ subscribers making or swearing a stat. dec. - statutory declaration and 2. 'I do (hereby) declare!' strikes me as more of an American (movie) turn of phrase.

Explanation:
In a legal context, I think this would probably be the most appropriate heading for a section like this.
"I hereby declare that..." seems to fit quite well.
