This is from a history and physical examination report of a patient who has had a melanoma removed through excision biopsy.

"... esito di pregressa biopsia chirurgica, clinicamente non sono apprezzabili noduli sottocutanei o adenopatie sospette. Il Paziente e' candidabile ad intervento di *radicalizzazione* nella sede del tumore primitivo contestualmente alla ricerca del linfonodo sentinella di pertinenza." Later: "In data [xxx] il paziente e' stato sottoposto a *radicalizzazione* nella sede del tumore primitivo ed asportazione del linfonodo sentinella ...." And again later "... si esegue ampia exeresi di radicalizzazione .... "

I've found an Italian description of the procedure, but it doesn't help me to translate the term into a specific English term:

La radicalizzazione è un intervento chirurgico eseguito in anestesia locale nella sala operatoria della Dermatologia (ricovero in regime di Day Hospital). Il chirurgo:
• asporta la cicatrice del precedente intervento (la biopsia);
• asporta una ulteriore porzione di pelle sana intorno alla cicatrice
(allargamento);
• invia i tessuti asportati in anatomia patologica per farli esaminare
(esame istologico).

To be honest, my feeling is that far more than a random proportion of articles discussing "radicalisation" or "radicalization" in context with melanoma, come from Italian, Polish, Czech authors.

Based on these articles, and in particular Luigi's and mrrafe's references, the definition seems to be clear: re-surgery which is performed more radical than the first one. "
Wide local re-excision" may be a way to express this concept. Native specialists may hopefully come up with more robust proposals ;-)

As liz found, "re-excision" seems to be sufficient and is used more commonly, see e.g.:

Because surgical excision is the primary treatment of biopsyproven melanoma at any site, the initial biopsy is critical in determining the appropriate surgical strategy, including the calculation of optimal surgical resection margins, and the necessity of a sentinel lymph node biopsy. These decisions are largely based on the thickness, ulceration status, and mitotic rate of the primary melanoma [3,6-10]. Re-excision is performed when residual tumor is identified in either peripheral or deep margins. In our study, 29.5% of cases with both positive peripheral and deep margins underwent further excisions. Overall, 14.6% cases had re-excision regardless of initial biopsy type after initial wide local excision.
