3 - Les différents types de délivrance
3 . 1 - La délivrance spontanée
Le placenta est évacué spontanément avec l’aide de quelques efforts de poussée de la patiente en position verticale.
3 . 2 - La délivrance naturelle
Toute délivrance qui n’est pas spontanée, ou dirigée par des moyens médicamenteux est considérée comme une délivrance naturelle.
3 . 3 - La délivrance dirigée
C'est une direction pharmacologique de la délivrance.

Son but est de raccourcir la période de la délivrance, en limitant ainsi les pertes sanguines, par l'injection d'ocytociques.

Actuellement, l'ocytocine synthétique (Syntocinon®), une ampoule de 5 UI injectée en intraveineux direct lors du dégagement de l'épaule fœtale antérieure semble être la solution de choix.

Il faut insister tout particulièrement sur une technique très rigoureuse, qui évite les principales complications reprochées à celle-ci (enchatonnement et incarcération placentaire). L'embol doit être effectué au bon moment (nécessité d'une tierce personne), en veillant à avoir le coude en extension, afin d'éviter un clampage anatomique de la voie veineuse et en accélérant le débit de la perfusion immédiatement, afin que l'ensemble du produit passe rapidement dans la circulation sanguine.

Les anglo-saxons associent à cette injection une traction « contrôlée » et continue sur le cordon.
La délivrance dirigée, autrefois recommandée dans les situations à risque, est à ce jour recommandée de façon systématique.

Il y a trois types de délivrance: la délivrance spontanée, la délivrance naturelle et la délivrance dirigée.
La délivrance spontanée est caractérisée par l’absence totale d’aide extérieure lors de la réalisation de celle-ci.
En ce qui concerne la deuxième, l’équipe laisse le déroulement de la délivrance se réaliser spontanément sans utiliser de traitement pour la favoriser, hormis éventuellement une traction modérée du cordon et un refoulement du fond utérin.


En ce qui concerne la délivrance dirigée, elle diffère de la précédente par l’administration d’ocytocine au moment du dégagement de l’épaule du nouveau-né. L’ocytocine a pour but de provoquer des contractions utérines qui accélèrent le processus de la délivrance, et ainsi, permettent de diminuer le risque de saignement lors de cette phase. Ce dernier type de délivrance est actuellement réalisé dans de nombreux établissements du fait de son intérêt dans la prévention de l’hémorragie de la délivrance.
