This is from a form for what looks like either the acquisition or collection of a medical device. After the section on the device details is a section on the purpose of the acquisition/collection of this device, giving details fo the patients age etc. The first line in this section states:

"Coinvolto: Paziente"

I am unsure the best way to translate this here.
