Could this actually be "alto custo" instead of "autocusto"?

Eu tenho problema de esclerose, e o outro laboratório que tem o meu remédio, eles vieram aqui, trouxeram o disparador para mim, sabe, tudo pelo SUS, aquele autocusto, né?

Porque o meu... o dela também é autocusto, e o meu também é autocusto.

Alto custo, for sure.
I've already used that service from the government ;)

I'd explain this as a high cost drug program sponsored by SUS - Sistema Único de Saúde (Brazilian Unified Health System), or something like that.
