From what I can gather your document is a notarial instrument.

In Mexico, at least, there is usually a section public documents identified as "Generales" where the "comparecientes" are required to state their full name, date and place of birth, citizenship, address, etc.
I frequently translate this as "Personal Information" (taken from Becerra's Dictionary of Mexican Legal Terminology) or sometimes "Particulars", because most times the text that follows reads something like "El/la compareciente, por sus generales, manifestó ser: de nacionalidad ... por nacimiento, de este domicilio, etc."
