The normal meaning of "v/" is "ved" in the meaning of "bruges for at angive den ansvarshavende eller handlende i en bestemt sammenhæng" (see link).

So "[name surname] og [name surname]" could be an official company name and the rest could refer to the owner, etc.

I suppose, by the way, that you mean [forename surname], not [name surname], as "name" refers to both the forename and surname, so [name surname] would mean for example "Peter Hansen Hansen".
