French term or phrase: affiliation multi-ligne
I'm translating some "golden rules" from a Belgian university's research department for any members of staff wishing to publish something, specifically about the "signature" of their academic publications, i.e. details to be systematically included such as full names of author(s), affiliations, address, etc. The section ends with a list of definitions:

Affiliation multi-ligne
Les auteurs peuvent indiquer leurs affiliations à plusieurs institutions en utilisant des indices qui permettent de faire des renvois aux différentes adresses : (ii) "n" renvois par auteur dans le cas de "n" institutions : on parle alors du "multi-ligne" : chaque auteur renvoie à plusieurs adresses (autrement dit, chaque affiliation est représentée par plusieurs adresses).

Only one hit for "multi-line affiliation" and it's in a French-origin document that looks like a translation.
