Not much point in getting the question re-opened, as there are no answers to grade (!) but multiple or multi-institutional affiliations would seem to be the way to go (cf. answer to affiliation mono-ligne).

Thanks, Alison. Unfortunately, there's also a section in the text itself, called "Multi-affiliation", under which come two sub-sections: Format mono-ligne and Format multi-ligne. It's all very confusing, as these definitions are supposed to clarify the text that comes before, yet this particular term is not actually used there! I suspect I'll use "multiple (or multi-) institutional affiliations", as used in the title of your link - also fits in with the answer to the mono-ligne question (single institutional affiliation).
