This is a survey for patients seen in the Emergency Department and it asks whether they remember what medications they take, whey, how, if they don't take them, why, etc.
It is RECALL I am having problems with. This is what I came up with, but it sounds horrible:

En mi opinión Identificación (o también "reconocimiento") del tratamiento se refiere tanto al tipo de fármaco como a su administración.
No veo necesidad de ser literal. Si el paciente es capaz de reconocer o identificar el tratamiento que está siguiendo es porque lo recuerda y sabe los detalles, y si lo recuerda puede identificar el tratamiento. El isomorfismo no deja cabos sueltos.
