Thanks for your input. But 回答書 is a reply to an employee's enquiry to ハローワーク if he's covered by the current employer. It's not a form to be filled in. The reason the employee needs to make such an enquiry is, although he is entitled to the employment insurance, being employed doesn't mean automatic coverage. It requires some administrative work on the employer's side. If they are slack, he won't be covered on his commencement day. In the worse scenario, they may even forget about his insurance. That's why he needs to make an enquiry to see if he is covered or not. (If he gets a 保険証, he doesn't need to worry. But some workers don't seem to get their 保険証 on their commencement day because their workplace (such as a construction site) is far from the company office although they are covered. What they worry is that they might not be covered because of the company's negligence.) That's what I understand about this 回答書 thing.

So, in rephrasing the suggestion made earlier both re: scenario and terms used, would be inclined to depart from the literal, to reflect more clearly as follows that particular insurance servicing obligation:

"Please use this form to confirm whether you are currently registered to service your obligations to the employment security insurance scheme."

So, setting aside one probable typo in your refocus, am taking it to mean:
" Following an employee enquiry [to the Employment Security Office] about his employer's relevant compliance status, this is their report form which that employer has to fill and return on whether he is duly registered to contribute his share of funding to the national employment security insurance scheme for his employees." right?

...it becomes clear that:

- a large number of small businesses are not compelled to contribute to the national employment stabilisation fund. So if one is considering joining one of these for the long haul, one had better check which category the business operation selected belongs to.
- past a certain size (to be checked), businesses are duty bound to a) make yearly provisions for their employees' entitlements for when they retire or have to be retrenched, and b) along with their employees, contribute their own share (50-50?) to nat.job.secu.insr.schm

Oops, wrong track! thank you for this refocus, Port City.
What threw me off track is the term 'eligibility'.
Eligibility connotes two things:
a) an option that opens up but which one is free to forgo, leaving it to others to avail themselves of.
b) the pay-off for taking it up is a benefit.

The situation we are now looking at (I had not read your initial explanation, I must admit. So, serves me right!) is very different:
this 資格 is no less than falling into a 'category' declared by law as being responsible for meeting a significant ongoing financial obligation, whether its its members like it or not, to a well defined section of the community, (in this case one's employees); and 資格取得 is committing to assume that responsibility that comes with being declared a part of that category.
