I think that this is continuing the metaphor of a journey as in "Voyage" "piloter" and setting up continuity is metaphorically illustrated as setting up the staging posts for changing horses, as it is literally is here:

Without context and the "style" of your PP presentation, it is very hard to gauge what is required (one reason I never translate PPTs!!).

I have used a phrase which does with "voyage".

Low CL here, there could be other solutions.

I believe it's either a metaphor for a ' pont basculant' , which needs to raise itself from either side to allow larger shipping cargo to pass, or 'raising the stakes' as in ' taking things to the next level';

Not quite sure how to phrase this since you'd need to know what the "relais" actually are. But I wonder if it isn't something to do with developing the role of communication/liaison staff/middle management within the company in order to bring about change.
