In a Brazilian marriage certificate, these appear under "Nome que cada dos conjuges (...) (Cuando houver alteracao)":

"O 1° nubente após o ato, chamar-se-á X"
"O 2° nubente após o ato, chamar-se-á Y"

I assume one is the bride, and the other bridgegroom [being opposite sex]. Just need to need opinions on whether "Bride/bridegroom name after marriage: X/Y" suffices.
