community involvement
Explanation:
At least in US English, this would be a good deal more natural than either "civic advocacy" or "civic engagement" and would seem to reflect what is being referred to by the Spanish term "incidencia cívica."

"Community action" and "community participation" would be other possibilities here.

effects/repercussions on the community

Explanation:
Hard to tell exactly what this means, but my view is this relates to how the centre affects/will affect (incide en) the community.

DRAE:
incidencia
1. f. Acontecimiento que sobreviene en el curso de un asunto o negocio y tiene con él alguna conexión.
...
3. f. Influencia o repercusión. La incidencia del clima sobre los productos agrícolas.

Peer comments on this answer (and responses from the answerer)
neutral  Robert Forstag: "Community effects/repercussions" would not seem to constitute "actions to be implemented" (the clear meaning of the Spanish introducing the items subsequently listed). / I think that the intended sense here is "having an impact on" (i.e., the community).

-> You're right of course and I should have pointed this out, thank you. This isn't an action that can be implemented, but then neither is "incidencia" in any sense of the word as I understand it.
