Correggo il tiro sulla mia proposta. Ho fatto una breve ricerca circa l'uso di unprovoked, idiopathic e idiopatico nell'ambito specifico della PE. Da quel che ho visto, ci sono tre scenari principali:

a) Senza cause né fattori di rischio conosciuti
b) Con fattori di rischio transitori
c) Con fattori di rischio cronici

Provoked viene usato talvolta con il significato b) + c) e talvolta solo con il significato b).
Unprovoked viene quindi usato con il significato a) o con il significato a)+c)
Idiopatico l'ho visto usare sempre con il significato a).

Esempi:
Unprovoked = a)
Provoked PE - PE where there is an identifiable risk factor that likely caused the PE
Unprovoked PE - PE where there is no identifiable risk factor that likely caused the PE

Unprovoked = a) + c)
provoked = following surgery, trauma, immobilisation (transient risk factors)
unprovoked = permanent risk factors or idiopathic

Quindi direi che la risposta corretta dipende da cosa si intende per unprovoked nel tuo testo. Nel primo caso direi che la proposta di Aldo (e tua) può andar bene. Nel secondo, direi che dovresti tradurre con con fattori di rischio permanenti o idiopatico (tutto insieme). In ogni caso, direi che la mia proposta non va bene.
