It's not really this bit that I am stuck with, I just can't seem to figure out the following sentence:

La problématique de l’observance des patients constitue un axe essentiel et majeur de la communication de l’Association XXX pour la maladie de Wilson à destination des patients et des familles, en étroit soutien avec celui des équipes médicales et paramédicales du centre national de référence pour la maladie de Wilson (CNR) et des centres de compétences (CDC).

Logically 'celui' must refer back to 'axe' but I just cannot work out this sentence!

My attempt:

The problems surrounding patient compliance constitutes an essential and major focus of the information sent to patients and families by the XXX Association for Wilson's Disease, closely supported with that of the medical and allied healthcare teams of the national reference centre for Wilson's Disease and the centres of excellence.

But the second half of the sentence doesn't fit with the first!!
