I think Dx and Sx are right but I'm not sure about 'organic mental'.

The two main kinds of mental illness are 'organic' and 'functional'. Organic mental illnesses – such as the dementias that result from Alzheimer's, Pick's, Creutzfeldt-Jakob's, Huntingdon's diseases, acquired immune deficiency syndrome (AIDS) and those that result from dependence on mood-altering substances – result from brain impairment and are relatively rare. Although these are often called 'mental', they could equally well be called 'physical', because the brain is a physical organ.

I wanted to include 'syndrome' or 'disorder' in my answer but eventually decided not to because a) neither word is in the original and b) there are results for 'organic mental symptoms'.

They recommend consideration of B12 deficiency and serum B12 determinations in all patients with organic mental symptoms.
