I do not know how to translate this as I am unsure on the exact meaning here. Does it mean "set up on this date"? It appears after a description of the new baby's (and parent's) particulars:

"Dressé le 20 avril 2015 a 9 heures 15 minutes sur la declaration du pere, qui, lecture faite et invite a lire l’acte, a signe avec Nous, ..."
