Mientras almorzaba, paciente presenta dolor toracico irradiado a region dorsal y hombros bilateral, con perdida de tono postural sin relajacion de esfinteres con perdida de conocimiento por varios minutos, con recuperacion posterior sin focalizacion, motivo por el cual es traido a urgencias.

In the physical exam notes, everything is normal (except for notes about heart disease), and the patient has nothing wrong with his eyes or mental status, so I have no idea what sort of lack of "focus" or "focalization" the text is referring to. Any ideas would be greatly appreciated.
