I'm tending toward "healthcare establishment", but have seen "hospital" used in similar texts.

I agree with your own suggestion; this form texts needs to be as general as possible, and 'hospital' would be too restrictive, other than in certain specific contexts. It is possible for there to be other 'establishments' that are not actually 'hospitals'
