This is an Application for Financial Assistance to cover their medical bills. Again, this is a disclaimer for a hospital. They want to make sure the applicant is telling the truth. I'm sure they also want to cover their bases in case of fraud. ONLY the last phrase is giving me problems. The whole paragraph reads as follows:
