you have two important parts:
-- the justification for the decision (obrazloženje odluke)
-- the decision itself (dispozitiv odluke)
I would suggest that depending of what follows, you select any appropriate formula used to introduce it.
for example:
considering the following: (za obrazloženje odluke) or
we have decided as follows (za dispozitiv odluke) (I think it's what it is, but can't be sure without the ST)

I can't see how in the text of decision, an authority would say "we put forward as follows" - that doesn't make much sense to me, that would be fine for putting forward a proposal, but this is the enunciation of a decision.
An authority would for sure decide, and if the decision is properly worded, there would also be a justification for it added.

If it's used in a similar way as here http://www.revizorska-komora.hr/pdf/Suradnja-s-drugim/Mišlje... it would be: "we have decided as follows"
