Hi all -

I am translating a medical report on a patient admitted via triage as an emergency case. In the report they are using a form listing all the test a patient might be subjected to - including some that were not performed as they didn't apply to this particular patient hence I still have to translate them. (Or they were performed and no results are reported.)

It is also filled with misspellings...

At any rate, here is the context: a short table with headings and entries:

Thanks so much! I needed to be sure this was what doctors would actually use in a form and that there was no outside chance it had to do with printing out any diagnostic testing.
