I have just done exactly the same one - but the catheterisation reports from Nitra are usually pretty much identical.
RCx = CB
Then IMO it is a typo underneath that - it should be JR 4.0 6Fr not JHR

disagree  Misha Smid: I don't think that a vein can become a homocollateral for an artery. These are two different systems. PLV means "posterolaterální větev".
