Patients with significant hemorrhage require replacement therapy and repletion of clotting factors. Fresh frozen plasma contains components of both the coagulation and fibrinolytic systems and may be given to attempt to correct the coagulopathy. Platelet transfusions are usually given only to patients with active bleeding or a platelet count of less than 20,000–50,000/mm3, but is controversial due to the potential to cause more clotting following administration. Cryoprecipitate may be given to patients with plasma fibrinogen levels below 100 mg/dL. The usual adult dose is 5 to 10 U, with each unit raising the fibrinogen level by 5 to 10 mg/dL. Administration of antithrombin III, which balances clot production, shortens the duration of DIC and can be replaced using heat-treated pooled plasma concentrates. Red blood cell (RBC) transfusions, although not useful for repleting coagulation factors, may be given to increase hemoglobin and oxygen-carrying capacity. The use of heparin to minimize clotting (thereby slowing the progression of DIC) is controversial because heparin also increases the patient’s risk for bleeding. Few clinical studies have shown heparin to be effective in patients with acute DIC.
