From an approval letter for a clinical drug trial, included in the list of referenced documents:

-Tarjeta de paciente para emergencias
-Presupuesto tipo
-Certificado de seguro
...

I'm not familiar with the use of "tipo" in this context. Any suggestions would be appreciated!!

This document comes from Chile, and it's for a U.S. pharma company.

"Budget Type" as in Type of Budget


Explanation:
It is correct to say "presupuesto tipo" in Spanish on writing (commonly seen in forms, cards, or formats that have limited space to fill out info.). It's just an abstract way of saying, "type of budget". Perhaps referring to how the patient may pay for service (i.e. insurance, out-of-pocket, etc.)...

However, the form is not specified. To my knowledge, that is not the name of a document but it rather indicates a financial document of some sort... (i.e. Budget Form, Budget Plan, Financial Form, Proof of Income) Someone from Chile may be able to clarify this specific line.
