Zur weiteren Verlaufskontrolle und zur Erhebung von follow-up Befunden im Rahmen unseres Aortenklappenregisters bitten wir die Patientin in unserer Klappensprechstunde vorzustellen.

I am looking for a good equivalent of Aortenklappenregister. Is this a book or a database holding info about patients undergoing this specific treatment? This patient has recently undergone a percutaneous transfemoral aortic valve replacement. Secondly, the term Klappensprechstunde is unclear. The sentence of course says that she has been called in for a follow-up at the consultant's clinic, but how would you translate KLAPPENsprechstunde? Valvular review clinic perhaps?
