disagree  Natalie Pavey (X): Unless this translation is specifically about Mali, I would not use the source term. Additionally, I do not believe it is good practice to leave words untranslated unless it is the name of an organization that does not function in the target language.

Explanation:
If this translation concerns West Africa in particular, where I lived for two years, I believe that commune directly translates as it is a local form of government (see my link below). I also believe that "cercles" directly translates in that it refers to discussion circles which are a common practice in West Africa. I would offer the translation: "... transferred from the State to local collectivities via communes and community circles as they pertain to health care."
