Heading of the certificate
I have found many references
Embalment and Sanitation Certificate
Closing of coffin (sounds odd)
A permit for burial and cremation

mise en bière is been discussed before as laying down of body in the coffin but I need precise heading (short too)

Please advise me the correct translation of above.

proces verbal de mise en bière
burial declaration

Explanation:
"proces verbal de mise en bière" is an official declaration that the body has indeed been buried.
it's not a death certificate, nor an embalming certificate it precisely refers to the official acknowledgment tat the deceased has been put in the ground. In English the term "burial declaration" is often used in this context, both officially and professionally. Good luck!

Alternative: contemporary record of placing the deceased in a coffin; formal record of placing the deceased in a coffin.
(Apparently the online discussion regards the time of the laying out as the time at which this record is produced.)

Explanation:
The following references make it clear that the "proces verbal de mise en bière" is not a burial certificate, as the bodies are about to be repatriated.

This presumably is necessary in order to verify that the coffin contains a body and not arms, drugs, gold etc. and the identity of that body, in order to prevent people from smuggling stuff in coffins while pretending to be grieving relatives.

Try googling a snippet of the text and that should give you the reference. It is too long for ProZ's system.

I would translate the "attestation de mise en bière" as a statement. Note also, that an "attestation" is an unsworn statement (see F H S Bridge). The document produced by the police is likely to carry more weight than that produced by the undertaker.

Re your proposed translation for "attestation", you are not supposed to piggy-back one question on another and should really enter that as a separate KudoZ question. I think you are wrong about it being translatable as "affidavit".

The advantage to you in posting a second question is that it might attract other answerers, who potentially could provide additional information/arguments. Of course, you would need to note the connection to this question.

Notes to answerer
Asker: Thanks B.D. Finch, your references are very helpful

Asker: www.qualif.manche.gouv.fr/.../Démarche et%... is not working

Asker: Thank you

Asker: If this is certificate, then what is attestation de mise en bière?...attestation is too certificate

Asker: As per http://photos.state.gov/libraries/mumbai/890/pdfs/disposition-of-remains-report-docx-6-11-2010-2.pdf Affidavit by the undertaker attesting to contents, the embalming and the hermetical casket sealing Affadavit= sworn statement. Though I agree with your last point , but want to be more clear .Thanks

Asker: B.D. Finch, I would have posted question but I had no doubt but your answer was clashing with my next term so I asked it here. Thanks for replying.
