Invece, la nostra "carta d'identità" viene chiamata "cédula de identidad", "documento nacional de identidad" o "carné de identidad" (http://es.wikipedia.org/wiki/Documento_de_identidad).

Se nel tuo testo sorgente, dopo la dicitura "Cédula de registro de identificación personal" segue il codice o numero della carta di identitá dei genitori allora puoi tradurre con "carta di identità". Ma, se per il contrario, si riferisce all'identità del bambino, allora penso sia più corretto dire "Documento di registrazione dell'identità".
