Most importantly:
“There's an element of sarcasm in the phrase 'my heart bleeds for you', which is entirely inappropriate for expressing legitimate sympathies.”

Personally I'd say that there is more than just an element of sarcasm: it's almost always an extremely sarcastic phrase.

For example, we might say “My heart bleeds for you” to an extremely lazy person who, after doing no work at all for many weeks, complains of tiredness after working for five minutes!

Seitt I'm afraid I have to disagree with you on this. In fact I believe this phrase can be found in some Persian poetry. I have personally used it in sarcastic mode too but it's a valid phrase which has the similar applications as its counterpart in English. But generally speaking it's commonly used in daily conversations.
Often in high level language of art when you want to express how far you share someone's pain or tragic loss. Frequently seen in classical Persian songs where a lover sings it for her beloved and his heart specially starts to bleed when they are not together or have no prospect of getting together.
