Hola:

Estas siglas, PTR, aparecen al final de un "medical certificate" de un Hospital de Filipinas en el que se expone el diagnóstico de un paciente que fue hospitalizado.
Firma el médico, y justo abajo de su número de licencia viene el "PTR No: 1378".
Yo en un glosario he encontrado que PTR puede corresponder a "Patient Termination Record".
Si es así y no hay otra posibilidad, necesitaría saber la mejor traducción en el español peninsular.
