He has smoked two packs of cigarettes daily for 32 years and has a chronic productive cough. There is slight tenderness over the left ischium and hip. Serum studies show a calcium concentration of 10 mg/dL, phosphorus concentration of 4 mg/dL, and alkaline phosphatase activity of 560 U/L. An x-ray of the skull shows a dense, thickened, enlarged cortex. An x-ray of the hip shows a sclerotic lesion close to the acetabulum.
