grind organ

Explanation:
As per http://www.answers.com/topic/grind-organ

"a musical instrument that makes music by rotation of a cylinder studded with pegs
Synonyms: barrel organ, hand organ, hurdy gurdy, street organ"

The person doing it is definitely called an organ grinder.

My apologies. The terms I offered refer to the man who operates the "macinino".
I could not find the combined terms "organ grind" in any dictionary and it makes no sense to me. With that in mind I would opt for "street organ" as offered by Marco
